このページでは、日本にほん・台湾たいわん・韓国以外かんこくいがいの場所ばしょで受験じゅけんした人のみ申請しんせいできます。日本にほん・台湾たいわん・韓国かんこくで受験じゅけんした方かたは、各実施機関かくじっしきかんのウエブサイトをご参照さんしょうください。

機関きかんを送付先そうふさきとする場合ばあいは、機関名きかんめいに加くわえて受うけ取とる方かたの氏名しめいも明記めいきください。
To receive the certificate at an organization or institution, please enter the name of the person in charge of receipt in addition to the name of the organization or institution.


受取人うけとりにんが申請者以外しんせいしゃいがいの場合ばあい、委任状いにんじょうが必要ひつようです。
A letter of proxy is required if a person other than the applicant is the recipient.

EEA（欧州経済領域おうしゅうけいざいりょういき）から申請しんせいされる方かたは、GDPRに対応したプライバシーポリシーに同意どういした上うえで、「再発行申請さいはっこうしんせいを行おこなう（Submit form）」を押おしてください。
EEAから申請しんせいされる方かたで16歳未満さいみまんの場合ばあいは、保護責任者ほごせきにんしゃの同意どういを得えた上うえで、「再発行申請さいはっこううしんせいを行おこなう（Submit form）」を押おしてください。
同意どういのない場合ばあいは申請しんせいできません。

Those who are applying from the European Economic Area (EEA) must read and agree with the GDPR privacy policy for users in the EEA before pressing “Submit form.”
Those who are under the age of 16 and applying from the EEA must obtain the consent of a parent or guardian before pressing “Submit form.”
